“The attorney that created this for me said this is a Utah State form. He modified the form for me inserting my protocols and wants.
The attorney informed me that this is a legal, binding document that doctors/medical professionals/hospitals must abide by. But… I am not naive enough to believe even with this document, that medical professionals won’t fight you every step of treatment; your advocate must for firm, forceful and not acquiesce to doctors. I had the Medical Directive created in case some unforeseen medical emergency forces me or my loved ones into the hospital.
I had a copy of the document emailed to me and had trouble opening it. I ended up “downloading” it for it to open, you might have to do the same. The Directive can be modified in pretty much any way desired. This reflects what I felt necessary at the time but things are changing everyday. For instance, I forgot to insert “No restraints” but your advocate can address this.
If anyone needs help in filling the form out I am more than happy to assist them. One of the benefits of having the attorney create the document was his explanations/meaning.
Please not: I inserted that “I am invoking “S204 Right to Try Act, signed into law 5/30/18 by President Trump allowing an individual to use non-FDA approved drugs.”
